What determines if I need to submit a Home Modification application to the Design Review Committee?

Any change to the exterior of your home, front yard/side yard/backyard/under the ground or up to the moon must go through the Design Review Committee. When replacing like for like (same species of tree in the same location or replacing broken roof tile with same color/style/texture etc. then that is considered maintenance and not a change. This rule was put in place by the developer before the first house was even built.
